Priciple Task: To analyze a PRESSURE TUBE RUPTURE at Pickering A
Problem Statement (as given): PRESSURE TUBE RUPTURE resulted in LOSS OF COOLANT ACCIDENT (LOCA)
To brief media
To brief employees
To brief neighbours
To fix current problems
To guide future design decisions
To achieve the above Goals the following Tasks must be completed:
General
(1) Describe the Pickering A NGS
(2) Relate it's design, capacity and features in general to other CANDU's
(3) Explain the importance to power generation in Canada i.e state what it's non-contribution would result in
(4) Describe the accident, literally and using a event timeline - specifically LOCA Event Timeline
(5) Prepare a Media Pack (a brief description of the plant w/info regarding the actual rupture and its implications for people)
(6) Obligatory statement to employees, whether to confirm or alay their fears - depending on findings
(7) Goodwill flyer (statement to the neighbours)
Technical
(8) Situation Analysis (SA) at the time of the event (analyze the operator's situation at time of failure)
(9) Analysis of the problem(s) producing the fault (PA)
(10) Analysis of how the operators corrected the fault (DA) and comment
(11) Analysis of how to prevent future faults (PPA)
Management
(12) Produce a group GANTT Chart outlining strategy for Pressure Tube Inspections @ Units 1 - 8
(13) Reactors built with limited lifetime, so develop suitable strategy for inspecting all pressure tubes
